| (22253) Date: 2019-05-20 Entered By: Suzanne Newton - (Caregiver) Type of meeting / Contact: Meeting at School Reason: Jess came in to share her concerns in regard to Cooper returning to school from stand down. He is feeling low and is extremely unhappy. She is lacking confidence in the school being able to support Cooper's needs. Feels the behaviours in Room 9 are difficult to manage although loves the relationship Cooper has with BI. Jess upset that MOE will not support Cooper, that he does not qualify for Reading Recovery and that the RTLB process is so slow. She is unhappy with the fact he is not eating at school, he is gluten free and they struggle with getting him to understand the importance of eating his own food - gets bad diarrhoea. Concern from Jess that she requested Cooper was not in SH's class last year. She has concerns about the interaction both she and Cooper have had. She has Cooper on the waitlist for a STAND village stay. Not keen on counselling or Mana Ake because she had a negative experience with them and her older son. Outcome: Discussed that Cooper is not high needs therefore MOE will not step in unless requested, also they will allow RLTB to do their work first. Can case share if RLTB deem necessary. RTLB is a process - I will check where the Referral For Service is at. Jess was upset that they closed Cooper at the end of 2018, SN explained the why of this. Also explained that they do not work directly with students more the teachers nowdays. Cooper loves toasties - its the only way he will eat his G/F bread. I've said that we can trial bringing the ingredients in and making his lunch to then eat - I put down 12.20 making time for 12.30 eat (I realise I got the break times wrong). He could then sit and eat it. Likes Marvel/DC Batman - make a 5 block reward chart for him, if he earns his reward he can do some gluten free baking - Jess to provide the packet of ingredients. Social Skills - he's not participating in the Circle time directly that I have witnessed. Are we able to look at how we present this to him? Other options? Reading - I'm willing to use some time in either Dianne's timetable or SEG to have him reading with an adult for fluency each day. At least twice a week would be good to try and boost him. I explained to Mum that the Lexia programme is operating as well. We will need to discuss how we accomodate the request regarding SH reading class. General Comment: There is a lot in here however we need to be seen to be taking action and supporting Cooper in the interim before RTLB does a pick up. Time: 9am |

| (22213) Date: 2019-05-15 Entered By: Raywyn Wright - (Caregiver) Type of meeting / Contact: Phone call Reason: SN asked RW to contact Cooper's family as he had gone from an in house to a formal stand down and she was in a meeting. RW rang Cooper's Nana, Mrs Smith, as she was unable to make contact with Cooper's mother and father. RW introduced herself and said we had met before at school. RW asked if this was a good time and explained she was calling because we couldn't contact his parents. Before RW could explain my reason for calling Mrs Smith became extremely defensive and started asking questions, have has got a teacher aide yet? RW replied no not yet, Mrs Smith then asked why not, he was accepted last term. RW explained that Cooper's RTLB referral was being processed and that the process included being presented at a meeting and if picked up by the RTLB then there will be observations, more meetings including the RTLB meeting with a parents. Then the RTLB meets with the teacher's offering strategies and skills to use with the child and also meets with a teacher aide to train them in programmes before they start working with the child. Mrs Smith said this is very unfair and not good enough considering it's those other boys in his class that are making him misbehave. Mrs Smith asked if it would make a difference if she rang the RTLB or the Ministry of Education. She offered to write a letter to her local member of parliament or to Jacinda Ardern. RW listened to her and then proceeded to explain the reason for her call, that we need someone to come and collect Cooper as he had been formally stood down. Mrs Smith asked what does this mean? RW said Cooper isn't allowed to returned to school until Friday. Mrs Smith asked what comes next? Will he be expelled? RW explained Cooper has only had one stand down this term not the 5 days in one term or 10 days in a year that would lead to Cooper going before the Board of Trustees before an exclusion happened. Mrs Smith said because of someone not doing their job my poor grandson is suffering. She asked what is going on in that class for this to happen and what is the teacher doing about it? Why should my grandson suffer? RW reassured Mrs Smith that we are doing everything we can for Cooper and that these things take time and they never happen as fast as we would like them too. Mrs Smith said she was on her way. |

Outcome: 1. Checked with Jordan the phone numbers on offer, note change to home line and he has requested that this is the number that is called in the first instance. 2. Explained Cooper's behaviour, he had sworn at Mrs Inglis in the morning session calling her a bitch in front of others. 3. Effort was made to speak with him however he was engaged in his learning so decision by SN not to interrupt or escalate further. Cooper arrived after 12.25pm escorted by Whaea Rachel he had been calling her a mother fucker from the top of the sandpit roof (unsafe behaviour that sets a dangerous precedent for others) Mr Green provided support to coax him down. Situation then moved to TJ, RLM and Cooper (Rachel to document this) where Cooper hit Whaea Rachel and she had to protect other students from harm. Cooper was placed on an in-school stand down for this behaviour so information could be gathered. He cooperated however took off at break time and hurt another student(s) using physical assault. RW had made the call to grandmother who was rather escalated. Jordan said 'Yes, I can imagine she was'. He agreed to do collect as it was now a formal stand down. General Comment: Met Jordan at the carpark with Cooper. Cooper was not happy about Dad being called, got really upset and began calling Mrs Newton names and tipping furniture. Cried in the car and would not put his seatbelt on. Agreed to hold the return to school meeting at a mutually convenient time. Time: 2.15pm |

Incident: A group of boys came to me on duty to say Cooper had punched Junior and hit Tyson Madden and kicked Ezekiel. Junior seemed a bit shaken. I felt Cooper needed to be out of the playground. I asked Barb and Tim who were handy to help me get Cooper in the office has he had hit 3 boys. They coaxed him in. At the end of break I took Ezekiel to R10 to discuss further. It seems Cooper was on the Senior playground and lashed out, in response to students yelling at him to get off. I assured Ezekeil that Cooper would be dealt with. Phoned Anna to let her know Tyson had been hit and she agreed tell him that Copper was being dealt with. Physical Violence: To Students Location: Afternoon break Action Taken: Passed on to SMT |

He was noisy when I came in had already hit or kicked KD in the knee. Raywyn came in to get him, he threw the book across the room he was reading and threw things in his way as he was leaving. NWP was entering the doorway into the classroom he hit her hard in her stomach. Swore also on way out. Cooper picked up the book he was reading and yelled at RW I hate you you fucking bitch and threw the book at RW. The book hit RW in the side of her face. Cooper did follow RW to the office however in the process Cooper thumped two girls in the stomach. BI to write this incident as she has the history. Threw equipment. Finally on mat laydown annoyed RC. At maths went straight to equipment without waiting for instructions was silly with this but did some work/math discussions with me. During morning called me a FB to my face in front of class Buttface numerous times very unsettled and non engaged with his learning. During this time rang SN as he refused to go on steps. At playtime I got his bag for him to get his morning tea. He got out 1 thing, but refused to come with me across to thinking time. After play he was annoying TJ he then raced outside screaming (for at least 5 mins at top of his voice). He gave me a 5 hand. I said at the end of this he needed to go to reading in R8. He walked in the door and threw the crayons and container while screaming 'I hate you fucking bitches' and started screaming. He then hid under the table. Cooper came out from under the table screaming 'I didn't do anything.' He then slammed the door open and closed. | (22215) Date: 2019-05-15 Entered By: Rachel Simmonds - (Tracking Form) Witnesses: RS Incident: Cooper was at lunch. He kicked Ricco in the leg. As RS went to step in, Ricco kicked Cooper. Both boys ran off, one chasing the other. Cooper told RS to 'fuck off'. Both wouldn't return. I eventually coaxed Ricco in who was worried about Cooper. Ricco sat and ate his lunch. Cooper climbed and stood on top of the tunnel on the playground shouting fuck you, mother fuckers etc etc. I had to leave only the TA with all other junior children as I tried to call SM, office and back office for assistance. Unfortunately, no one was there. Cooper then climbed up on the roof of the sandpit. He wouldn't come down. I found SG and asked him to help. SG managed to coax him down and Cooper ran away. I left it at that as I couldn't leave the junior area. At the end of break I popped into Hapua to let them know. As I walked through the backdoor, Cooper was there with Ricco and Tyler. Both boys seemed worried because Cooper was going to get them. I said no leave them. Cooper ignored me and was going to go after them. I stepped in the middle and put my hand on the wall to block the pathway to the boys and he punched/whacked me with his hands several times in the back/side/arm and shouted don't touch me. I said do not hit a teacher. He went after boys and I told them to come close and I put my arms around them. At that time SH was coming and I asked her for some help. SN follow up by coming over, Cooper was sitting on the mat, in the learning however calling out. Spoken to and asked to come to see Mrs Newton, aware that drawing him out of class may escalate so left him on the fringe. Monitor as he may come around, engage as needs to. Cooper needs to understand that he needs to behave in a respectful and responsible manner as a learner in the classroom. BI has organised a family meeting. Other: Other Location: In class |

BI reported that CCC was swearing directly at her in front of other students. He was non-compliant with a bag on his back. Door to the cloakroom had been locked and he had gone out to morning tea. SN , RW both dealing with other behaviours, no call received for support. At 11.30, SN with another student visited the learning space and CCC was engaged positively in a reading group session. Informed BI that I would not escalate him by removing him at this point however discussion needed to be had regarding his behaviour. Email received at 11.45am Cooper left the class at 11.40 just after you walked through Unable to follow up as with another student and heading into meeting. Please record and we will discuss with Cooper 1. Behaviour Plan where possible - BI as behaviour was at you could you arrange and SLT will follow up. 2. At meeting with Jess - Cooper taking responsibility and what is acceptable/not acceptable eg. swearing at a teacher 3. Remember that pre-12.30pm medication he will be on a low and not able to function as effectively. This is where low demand activities should be in place however behaviour needs not to impact on others.
