Victim: Jazmine Hapimana Young - Female - Rm8
Witnesses: Victim - Zoe Green Street Witnesses - Rachel (TA) - first half of the day. Ruth (TA) in Room 8 for all of third block. BI teaching next door. Andrew and Suzanne.
Incident: I sat and talked with Dakotah before school to give her an idea of what was happening. Dakotah seemed calm until the bell rang. She said she was hungry so I helped her to choose something healthy to eat (apple or banana). She refused to come to the mat when invited and began yelling 'no, no, no'. She began running in and out of the classroom screaming so I rang office for help. Andrew sat with her while we did our class meditation. When Andrew left Dakota yelled. 'I'm hungry and went out to her lunch box and came back in with chips, bar and crackers. She walked around the room. She then began walking in and out of the classroom and would not settle. Called office. After play Dakotah came and settled on the bean bag with a book so I praised her and gave her a token. Dakotah got up again and began wandering around the room yelling. Rachel (Teacher Aide) came in to get Dakotah for reading and Dakotah refused to go and began screaming. She eventually went with Rachel to Room 10 and a short while later I ealked to the Room 9/10 door to check she was settled. I observed both RL and Dakotah playing at the door way, yelling and disrupting the lesson. I asked the reliever if she wanted me to ring for help and I did from the Room 8 phone before going back to teaching my own students. After lunch eating Dakotah came back into Room 8 at her own accord. Ruth (TA) was also in the room to work with ESOL students. When we began the writing lesson Dakota began yelling 'I want to work with Mrs Jackson'. I explained that she could sit with me at the jellybean table and be part of the group I was working with. She repeatedly yelled, 'no, I want Mrs Jackson'. I told her that once she had finished her work she could share it with Mrs Jackson. She then noticed that someone else was sitting on the chair that she had tied tinsel around yesterday and screamed at the child to get of her chair. I asked the child to move to another chair at the table so that Dakotah could sit and begin to write. Dakotah sat but refused to write. Instead, she got up and dragged the chair around the room saying it was her dog. As she went passed Zoe she threw Zoe's writing book on the floor. I most of the tinsel off the chair and asked Dakotah to sit beside me. She retied tinsel and continued to drag her chair calling out and interrupting others learning. RH (teacher aid) cut remaining tinsel off the chair and tried to focus Dakotah. Dakotah yelled 'I'm out of here.' and began walking in and out of Hapua rooms. I rang the office to let them know as Ruth said that there was a digger outside Room 13. A short while later Dakotah appeared and said that Mrs Jackson had sent her back for her work. I shared with Deb that Dakotah and left of her own accord and she spoke to Dakotah saying that she needed to stay in Room 10. Senior managament came and got Dakotah and her writing book and took her over to the office. At 2pm some seniors came to tell me that Dakotah was outside the learning centre. BI also told me later that Dakotah appeared in the library. I rang to let the office know that she was outside. Dakotah came back into class and began tipping over chairs. Students were finishing off work and working on Lexia. Dakotah picked up cushions and began throwing them at students. She then hit Jazmine 3 times with force on the head with a cushion yelling 'I hate you' and did the same to Zoe. I moved both Jazmine and Zoe to a space behind me out of Dakotah's reach and rang the office. Dakotah stopped hitting with the cushion and began to throw things off my teacher easel. She pushed stuff of the easel shelf and began to climb in and out of this space. Suzanne and Andrew arrived to speak with Dakotah and took her to the office. Suzanne said that she would contact Nicky. Throughout the day I spoke to Dakotah in a calm and caring voice. I tried to engage her in each lesson with activities that were at her level. For the majority of the Day my interactions with Dakotah were witnessed by Rachel, Ruth, Barbara, Suzanne and Andrew. I have also created a calm down box to help Dakotah to manage her feelings but I have not been able to implement this today.

Incident: Unsettled at the start of the day. With encouragement she came to the mat. When we got up to go to assembly Dakotah began yelling 'no' no' no'. I asked chn to line up for assembly and and she stayed in the class yelling 'no I won't'. I saw Viola and in Room 10 (Raywyn was teaching) so I asked Viola if she could keep an eye on Dakotah. I planned to settle my class in the hall and hen come and get her. By the this time Dakotah was at the hall door and I encouraged her to come in and sit down. I sat beside her on the floor and used the tokens as rewards for sitting and listening. After assembly we returned to class to do our spelling test. As mindfulness had worked well yesterday when I said that I would lie beside her and hold her hand. This did not work. Instead Dakotah moved between people. I ignored her and she began touching people and saying I am hungry. I got her lunch box and told her that she could have some food. She picked up her mandarin and threw it across the room. I asked her to go to the thinking space and gave her several books to read. This was all while kids were meditating. She refused to stay int the space so I took her to RW (Room 10). RW queried where I was taking Dakotah and asked if I had worked through the steps. I reassured her I had. RW said she was already looking after a Step person so I took her to JB. DMcR returned at the beginning of play and refused to go to the hall practice that SN had organised. I asked SN if her tokens could count and Cool Kids cards so that she did not need to do the practise and SN said she needed to be there. Eventually Dakotah came to the practise. After play Dakotah had the iPad time that she had earned. Towards 12 oclock I asked the kids to pack up and Dakotah began Yelling 'penis' and sigling out both JY and ZG yelling that she hated them and that they were ugly. I asked the class to get the lunch boxes and sit on the seats outside Room 8 until it was 12 and they could go to lunch. Dakotah walked in and out of the room slamming the door. Eventually, when the kids had moved down to the eating area, Dakotah came inside and began to tidy up before sitting on the mat. We talked about what was happening after the 12.30 bell (discovery time) so that she was prepared. She said that she would sit on the mat, up the front, and help to get ready for discovery. I gave her her lunchbox and she went to lunch break.

Incident: Dakotah before the bell and asked if she would like to talk about the tokens I had made. I showed her the tokens and she aid that her photo looked pretty and I agreed. Together we brainstormed behaviours that would earn the tokens - doing as you are asked straight away, sitting on the mat, starting work straight away, using kind hands and feet. When the bell rang Dakotah was reluctant to sit on the mat. I told her that she would earn her first token. She refused to sit down and began to mutter and walk around the room. I took the roll and we began a mindfulness activity. I lay down on the mat to join in the class and felt/her other voices, as Dakotah walked around the children. I sat up as I was worried about what was happening. We began maths as a group and Dakotah went out to the cloakbay and reappeared shortly with biscuits from her lunchbox. She said that she had had no breakfast and that she was really hungry. I looked in her lunch box and she had a substantial healthy lunch morning tea. I rang mum who said that Dakotah had eaten a big bowl of Nutrigrain. I asked to choose one thing and come and sit with us on the mat. She chose to walk around the room yeling no, no, i'm not going to. I ignored her for 10 minutes and noticed Dakotah when she returned with more food. I took her lunchbox and told her that I would look after it until playtime so that she could join us. She keept yelling. SN walked through class and observed this and asked Dakotah to come over to the office with her work. Dakotah refused and SN siad that she would give her 5 minutes to think about this and would collect her on the way back. Dakotah continued to behave in this way until spelling at 10.15, At playtime Dakotah worked on her maths activity in the office.

Incident: Dakotah was happy and upbeat after play. She sat with ZG who read her a book. Lots of praise (token given), she came to the mat when asked (praise and token given). We moved to the library and began to choose our library books. Dakotah became unsettled and walked around the room yelling and hicking out at people. I was not able to get SN, and forgotting RW was in the office. I rang Room 10 for help. VK said she would ask JB to come over to help. I thanked her. JB returned Dakotah at the end of the day and said that Dakotah was worried about not having friends. We talked about needing to be kind to make friends and being able to make good choices so that others were comfortable and happy to be friends.

Type of meeting / Contact: Meeting at School
Reason: I saw Ripeka at playtime during our crosscountry day and I spoke to her about Roman's willingness to complete work in reading yesterday. I also spoke his behaviour in writing where he refused to write unless I worked with (see Steps notes). Ripeka raised the issue that he find some learning hard. I agreed with her but said he now needs to be willing to persevere and remain focused on tasks until they are completed to the best of his ability.

I also spoke about his organisational skills e.g. trouble following insructions e.g.to stick work into his book. and then finding pieces of work looise in the classroom, or ripped put of his books. Ripeka agreed that he finds it hard to look after/manage his property and referred to lost school jersey's and loosing his shoes just a few days after she had brought them.

Outcome: Ripeka will speak to Roman at home about the importance of doing his best. I will find opportunities to teach the skill of perseverance in class.
General Comment: Ripeka is very supportive.

Incident: Over the course of yesterday and today Roman has made some poor learning/behaviour choices. These include refusing to follow instructions, talking back, mimicing the teacher, not completing work, ripping up work. As a result of this ongoing behavior he had time out to complete a behaviour plan and to copy the end of his poem into his book that he had dictated to the TA. Instead he refused to complete his behaviour plan and ripped up the poem ending so that he did not need to copy it. Raywyn spoke to Roman about his behaviour. I will give him the choice to finish his poem in class or back in thinking time. I will also ring Ripkea to follow up the behaviour plan.

Reason: Rang Ripeka to say that there was a new unnamed jersey, like the one he was wearing yesterday, left near his cubby. She said that if it was a size 10 it was Seidyn's.
Outcome: I named it and passed it on to him.
General Comment: Ripeka is happy with Roman's progress and improved attitude in reading and writing. She is still concerned that he does not seem to be making friends.

Reason: Rang Ripkea to touch base and to let her know that Roman is showing a much more positive attitude over the last week. I am encouraging him to see the connection between his attitude and his improving confidence and gains in reading and writing, A few of his classmates have even commented on his smiley/friendly attitude too.
Outcome: I will ring Ripeka again next week. In the meantime praise, praise, praise.

Reason: I rang Ripeka today about Roman's non-compliant attitude and behaviour in the classroom on Friday. Ripeka said that Roman had come home and told his Mum that he had spoken back to me and they had had a discussion around needing to use the school values and manners. Ripeka has taken Roman's scooter off him for the week as a consequence.

While they were talking Roman complained of not having any friends and his feeling that he is being bullied. Both he and his brother spoke to Ripeka about Jazmine being mean to them, swearing and pulling the naughty finger. I assured Ripeka that I would let Senior Management know of her concerns regarding Jazmine.

Outcome: Ripeka to remind Roman to use his manners at the beginning of each day.

I will ring Ripeka in a few days.
